msc host: complete and fence a command whose stage fails to submit - #4009
In msch_xfer_cb(), every stage submission (command → data, data → data, command/data → status) was a TU_ASSERT. When usbh_edpt_xfer() fails, usbh ignores the return and leaves nothing pending, so complete_cb never runs and the caller waits forever. This dates from before #3968; that PR's split data stage adds one more submission of the same kind. CodeRabbit raised it on #3968.
b3b8bed: a failed submission now completes the command with a synthesized CSW: PHASE_ERROR, the CBW's tag, and residue = bytes not transferred.
c21f94f: after that, the device may still be in the middle of the command. If the host sends another command, the device can read the CBW as data: a failed WRITE10 then writes it into the sectors. tuh_msc_scsi_command() and tuh_msc_ready() therefore refuse the interface until it re-enumerates. The flag is set before complete_cb, so a callback that re-issues a command is refused too. Bulk-Only reset recovery (§5.3.4) would lift this without re-enumeration; tuh_msc_reset() is still #if 0 and is tracked in msc host: no BOT reset recovery, example hang on submit failure, enumeration left unfinished on failure #3985.
Stacked on #3968, whose data_stage_xfer() this change needs; merge that first.
Validation
test_msc_host_data_stage_chunk_submit_fail_completes fails the second 0xFC00 chunk submission. It checks that the completion arrives once with the right CSW, that the in-callback retry is refused, that nothing else is submitted, and that close plus re-mount recovers. Each commit's new assertions fail on its parent.
pre-commit run --all-files passes. check_build.py --scope src/class/msc/msc_host.c builds 28 of 29 boards; rx65n_target fails the same way on master.
Size on stm32f407disco host/msc_file_explorer: +60 B flash, 0 RAM.
HIL host/msc_file_explorer: mimxrt1064_evk passes. raspberry_pi_pico fails the same way on the base. A failed submission can't be forced on hardware, so that path is covered by the unit test only.
The MSC host now completes commands when it cannot submit a data or status stage. It creates a phase-error CSW, marks the interface desynchronized, and rejects further commands until re-enumeration. Enumeration reports configuration completion and leaves the interface unmounted when specified operations fail. The GET_MAX_LUN callback ignores results for an unconfigured interface or a mismatched per-open tag.
Possibly related issues
hathach/tinyusb#4270: This issue concerns the set_config() completion contract. This change reports configuration completion when enumeration aborts.
hathach/tinyusb#4315: This change handles refused CSW status-stage submissions, but does not implement the requested endpoint halt recovery.
Merge Risk: Minimal · up to 6baa0
Failed MSC stage submissions now complete and fence the interface. No actionable merge-blocking risk was identified.
Security architecture risk: Moderate · up to 07c4b
A failed transfer is now completed and fenced, reducing the risk of sending another command to an out-of-sync device. There is a conditional gap in the separate enumeration-failure path: the host can report the USB device as mounted while the MSC interface remains unmounted but still accepts raw SCSI commands.
Retained concerns
Medium · security · inferred: Enumeration abort can advertise a generically mounted USB device while leaving its failed MSC interface configured and eligible for raw SCSI submission. A caller using the generic mount event and raw API can therefore send commands despite MSC initialization having failed.
Security review details
Security Blast Radius
inferred — The demonstrated boundary is the affected USB device’s MSC interface and its host-visible mount state; the supplied evidence does not establish an application that sends raw commands after this failure.
Security Findings and Attack Paths
inferred — A USB MSC device can report a failed Read Capacity status. The new abort path can then finish device-level enumeration; if a trusted caller responds to the generic mount event by using the raw SCSI API, the configured-but-unmounted interface can still submit a CBW. Application use of that sequence was not verified.
Trust Boundaries and Controls
observed — The desync fence is set before the failed command’s callback, so callback retries are rejected. Readiness and high-level read/write operations also reject an unmounted interface; those controls do not impose a mounted requirement on raw SCSI submission after enumeration abort.
Resilience and Maintainability Implications
inferred — The command-failure fence addresses the risk of a subsequent CBW being received as data by an out-of-sync device. Enumeration failure follows a different lifecycle: completing host configuration does not itself revoke raw-command eligibility.
Hardening Proposals
proposed — Make failed MSC enumeration an explicit ineligible state for external raw commands while preserving the commands needed during enumeration, and distinguish interface failure from device-level configuration success for consumers.

Public header and docs (msc_host.h, docs/reference/class/msc.rst) do not describe the new permanent-refusal behaviour or the host-built PHASE_ERROR CSW after a stage fails to submit.
msch_set_config returns false after it has already reported completion via config_abort, unlike cdch_set_config; harmless today because usbh ignores the value.
The unit tests have coverage gaps (Get Max LUN setup packet unchecked, NULL-callback path no longer reached, discarded return values) and minor style and fragility issues in test_msc_host.c.

src/class/msc/msc_host.h:55: low: The public header and docs do not describe the desync behaviour.
Once desynced, tuh_msc_scsi_command and its wrappers return false and tuh_msc_ready stays false until msch_close (msc_host.c:156, :340); the callback gets a host-built PHASE_ERROR CSW (:383-392).
Fix: update msc_host.h and msc.rst.
Update: the docs, msch_set_config's return value, the NULL-callback test and the return-value asserts are addressed in edff542. The stm32f723disco HIL failure also fails on master.

nit: The Get Max LUN test never checks the request's setup packet.
test/unit-test/test/host/msc/test_msc_host.c:44-47: the tuh_control_xfer stub stores only complete_cb. A wrong wIndex, bmRequestType, bRequest or wLength would pass, so ITF_NUM = 2 does not prove the interface number is sent.
Impact: coverage gap only; the driver builds the request correctly today.
Fix: record the setup packet in the stub and assert its fields.
The reason will be displayed to describe this comment to others. Learn more.
nit: New declarations and assignments break the .clang-format alignment.
test/unit-test/test/host/msc/test_msc_host.c:31 and :34 (complete_csw, ctrl_complete_cb) misalign the declaration block, and set_config_count = 0; in setUp (~:169) misaligns the = column.
Impact: cosmetic; clang-format is not enforced in .pre-commit-config.yaml.
The reason will be displayed to describe this comment to others. Learn more.
low: The tests pin a new API contract that the header and docs do not describe.
assert_submit_fail_completed() (test/unit-test/test/host/msc/test_msc_host.c:221-239) fixes it: after a stage fails to submit, the callback gets a host-built PHASE_ERROR CSW, and tuh_msc_scsi_command(), read10/write10 and tuh_msc_ready() return false until re-enumeration.
msc_host.h:42, :56 and docs/reference/class/msc.rst:96-98, :115-117 still say otherwise, so apps cannot tell a stuck device from a busy one.
Fix: document the permanent refusal and the host-generated phase error.
The reason will be displayed to describe this comment to others. Learn more.
nit: cbw is declared far from its use.
test/unit-test/test/host/msc/test_msc_host.c:285 declares cbw (.tag = 0x5678) about 12 lines before its first use at :296, ahead of the re-enumeration block. assert_submit_fail_completed declares its own local of the same value at :235.
The reason will be displayed to describe this comment to others. Learn more.
nit: xfer_count is not reset before re-enumeration, which leaves the stub's transfer table exactly full.
test/unit-test/test/host/msc/test_msc_host.c:291: xfer_count is 3 after assert_submit_fail_completed(3, ...). enumerate() then uses slots 3-7, so MAX_XFERS=8 is full. One more enumeration transfer would trip TEST_ASSERT_LESS_THAN(MAX_XFERS, xfer_count) at :82.
Impact: the test passes today; fragile for future changes.
Fix: reset xfer_count before mount_bot_interface().
Fence Get Max LUN completion by enumeration generation.
When another asynchronous control transfer owns the slot, msch_set_config() queues Get Max LUN. After close, the host marks that entry stale but still invokes its callback. A detach/attach can start a new enumeration before the queue drains. The old callback can then pass configured, set the new interface's max_lun to 1, and submit another Test Unit Ready command against the new enumeration.
Store an open-generation token in xfer->user_data and require it to match before processing the callback.

Valid in a narrow window, and fixed in cabbbdc; our earlier refutation was wrong. usbh fails a stale queued Get Max LUN at dispatch (src/host/usbh.c:1165-1178) but still calls its callback. If the host task runs an enumeration after_cb (usbh.c:891-898) before the FIFO drain (usbh.c:909-915), the stale entry can outlive msch_set_config() on a reused address and reach the callback with configured set.
_msch_tagid[] (src/class/msc/msc_host.c:67) is bumped per open (:445) and kept across msch_close().
Get Max LUN carries it as user_data (:476); config_get_maxlun_complete() returns on a mismatch (:491).
test_msc_host_enum_stale_get_max_lun_after_reopen (test/unit-test/test/host/msc/test_msc_host.c:518) completes a stale request after a reopen: no transfer submitted, no usbh_driver_set_config_complete(), max_lun untouched, and the reopened interface still enumerates.
