Add fault trapping, profiling workflow, and ITM test coverage - #64

- add fault trap configuration to run, simulation, and GDB paths
- map fetch/vector/stack faults to architectural exceptions and report trap context
- add fault trap matrix coverage and hook it into internal test runs
- split hello_world_itm into its own test fixture and validate ITM output in test_gcc.sh
- consolidate test linker scripts through a shared common.ld
- refresh README and TODO notes for fault trapping, ITM, and semihosting status
